Your smoker is holding 275°F. Your wireless thermometer's ambient reading says 205°F. So you crank the fire to "catch up," and an hour later your brisket has blown past where you wanted it and the bark is scorched. The reading was not broken. It was measuring the wrong pocket of air. Here is the physics behind it, why it is worst on low-and-slow cooks, and how to read your real pit temperature.
A wireless thermometer's ambient sensor sits on the Stick, inches from the cold meat. Cold meat is a heat sink: it wraps itself in a pocket of cooler air that gentle smoker airflow never clears. The sensor reads that cool bubble, not your pit, and can run 50 to 75°F low. It is not broken. It reads the wrong place.

When you put a probe in a cold roast and set it in a hot cooker, three things fight each other:
The result is a reading that can sit far below your true pit temp early in the cook, purely because of where the sensor is, not because anything is wrong with it.
The size of the gap depends almost entirely on airflow and how cold and big the meat is.
That pattern is why this problem defines low-and-slow BBQ specifically. The exact cook that most depends on a steady, accurate pit temperature is the one where the on-probe ambient is least able to give it to you.
Quick Tip: The bigger and colder the cut, and the less air is moving, the further your on-probe ambient will read below the real pit temperature.
Here is the part that reassures most people: the gap closes on its own. As the meat heats through, it stops sinking so much heat out of the air around it, the cool bubble shrinks, and the ambient reading climbs toward the true pit temperature. By the back half of a long cook the two are usually close.
So the correct mental model is: an on-probe ambient reading that starts far below your pit setting and slowly rises is behaving exactly as physics predicts. It is not a sensor warming up or a bug. It is your meat warming up.
The dangerous move is treating that low early reading as "my smoker is too cold" and cranking the fire to make it climb.
Your pit was at temperature the whole time. When you add heat to chase the bubble, you overshoot the real pit temperature badly. Then, as the meat warms and the bubble disappears, the ambient reading jumps up to meet a pit that is now way too hot. The brisket races through the cook, the fat renders unevenly, and the bark burns. Every part of that came from trusting the wrong number and reacting to it.
Quick Tip: If your pit thermometer and your on-probe ambient disagree early in a cook, believe the pit. Do not add fuel to close the gap. The gap closes by itself.
You have three options, in order of how reliable they are.
The on-probe ambient is not the only temperature on your rig that misleads you. On a typical smoker, three separate readings each miss the true grate temperature for their own reason:

The lesson under all three: the only temperature that decides your cook is the grate temperature, right next to your meat, out of the cold bubble. Not one of those three is reading there.
If the problem is that the ambient sensor is stuck in the cold bubble at the meat, the fix is to move the ambient measurement off the meat and onto the grate.

Why does my wireless meat thermometer read lower than my grill temperature? Because its ambient sensor sits on the Stick, right at the cold meat, inside a pocket of air the meat has chilled. It reads that cool bubble, not your cooker. In a smoker the reading can be 50 to 75°F below the real pit temperature early in the cook.
Is my ambient sensor broken if it reads way below my set temperature? Almost certainly not. A low early ambient reading that slowly climbs is the expected behavior of any ambient sensor mounted near cold meat. The gap is caused by the meat, not a fault, and it closes as the meat warms.
What is the heat-sink effect? A large cold piece of meat pulls heat out of the air touching it, wrapping itself in a layer of cooler air. Any temperature sensor sitting in that layer reads cooler than the rest of the cooker. The meat is acting as a heat sink.
Why is the gap so much bigger in a smoker than in my oven? Airflow. An oven or air fryer has a fan actively mixing the air and clearing the cool bubble, so the sensor sees close to the set temperature. A smoker has gentle airflow and a big cold mass, so the bubble sits undisturbed and the gap is large.
Should I turn up the heat to make the ambient reading match my target? No. That is the classic mistake. Your pit is likely already at temperature. If you add heat to chase the low ambient number, you overshoot, and when the meat warms and the bubble clears, the reading jumps to a pit that is now far too hot.
How do I know my real pit temperature? Measure at the grate, a few inches from the meat and out of the cool bubble. A wired ambient probe clipped at grate level reads the true cooking environment. The lid dome gauge does not, and neither does the on-probe ambient early on.
Does the internal temperature reading have this problem too? No. The internal sensor is inside the meat, measuring the meat, which is exactly what you want. Doneness readings stay accurate regardless of what the ambient is doing. When ambient confuses you, trust the internal number for doneness.
Do all wireless meat thermometers do this? Any thermometer with the ambient sensor built into the probe at the meat does, ours included. It is physics, not a brand flaw. The fix is to move the ambient measurement to the grate, which is why MeatStick's V Forge uses a separate Wired Ambient Probe there.
How long until the ambient reading catches up? It depends on the size of the cut and your pit temperature, but the gap shrinks steadily as the meat heats through and is usually small by the second half of a long cook. Watch it rise rather than reacting to where it starts.
Is this the same reason my grill's lid gauge is wrong? Related but not identical. The lid gauge is wrong because it sits up in the lid instead of at the grate.
